HEAP stands for Home Energy Assistance Program!
Things to bring when applying for Heat Assistance:

Proof of income, proof of total monthly income for all household members.
Proof of residence : Rent or mortagage payment, or copy of lease , or homeowner's insurance policy.
Proof of Household size: Birth cert., school record or Social Security card for each household member.
Proof of qualified alien status if you are not a U.S. citizen.
Copy of your most recent Keyspan Energy and LIPA bills.
Final Termination Notice "If you have received a utility shut-off notice."
